父母申请税号，W-7 form

我替我岳父母申请税号, 填W-7表有几个问题；

1)我选的是 D , Dependent of U.S. citizen/resident alien我是否应该把我和我太太的名字和社安号都写上。

2) 有一段填表说明，看不明白，是否我报税表的时候，要附上W-7。但是我现在是想先申请税号。
