If there is a court order, it must be followed, regardless what his mental state is. You can always modify the custody order but until then, you have to follow the current order.

If you feel his mental state is such that it would harm the kids, you can get a temporary restraining order/preliminary injunction and/or temporary custody order to limit the visitation, meanwhile, start the process to modify custody.
