请教孩子办护照的问题， 到哪里能拿到certified custody order?

终于离婚了，孩子的父亲放弃了所有的父亲权利，签署了terminate agreement.我拥有孩子所有的监护权。两周前去给孩子办理护照，今天接到一封信，要求： Submit a certified custody order.

这是什么证件，去哪里能拿到呢？我只有离婚协议书和父亲放弃权利同意书。
