老猫请帮忙 - 更改visitation arrangement on divorce decree,

which form to use? 每年都有这个问题,很讨厌. 前夫自己搬去住别州, 离开孩子,三四年来从来不看孩子. 但现在孩子要回国过暑假两个月, 他跟我过不去, 一定要孩子跟他暑假一个月. I have sole physical custody, joint legal custody, tie break vote on extra curriculum. 但divorce decree 上有写他可以combine x'mas visitation and summer vacation into one month.

1. 我可以强行送孩子回国吗, 他可以告我吗? (我已经告诉他孩子们回国时间, 他先前同意,后又反悔)

2. 我想跟court file petition to modify visitation schedule, 因为他并不履行visitation. 我找遍了lawlibrary, 但不知道哪个form to use. 能否告诉我应该什么样的步骤?

3. 如何可以拿到court consent for their passports?

4. 他有alcohol problem, DUI, 经常email and harass me, 我如果想改成sole legal custody, 有可能吗?
