问一个提前取401K的问题

我离开美国, 想提前把401K取出来. 如果现在取, 取出的钱要交税. 是算09年还是10年的Income? 好象还有penalty 10%. 我该如何付penalty? 是用支票向IRS付吗? 银行会给我哪种单子证明我取了多少401K?
