1. My tax return for Year 2008 (joint with my husband).
2. My recent pay stubs for the past 6 months.

1. two checks with a total of $1070:
a. Check 1: $985 filing fee.
b. Check 2: $85, biometric fee.
2. Two 2"x2" photos.
3. Copy of Form I-94.
4. Original copy of my father’s birth certificate.
5. Copy of my father's passport, including the pages with nonimmigrant visa.
