or I need to add the following for last 5 years from 2007 to 2012?

Begin with your current or lastest employers and then list every place you have worked or studied for the last five years. if you need more space, use a separate sheet or paper.

My question is, during five years, I have worked for 3 years from 2009 to 2012. and from 2007 to 2009 I was a housewife and staying at home. Could you please let me know how I can fill out this for the right answer?

Only your working experience, not your house wife experience.
