1) I just found my birth notory from China, however, I had my nick name on te notory which I never used as any legal document,

-If your birth certificate has your formal name, that may be OK. The nick name does not matter.

 and my place of birth was different that my passport,

-What do you mean there were two different birth places? Passport normally only shows the Province name (e.g. Guangdong Province) and the birth certificate include the county name and the province name (e.g. Zhongshan County, Guangdong Province). If your case is the same as this example, you will be fine.

hence, different than the previous forms I have filed, ie i485, how do I fix this, is the letter with explanation sufficient, or I have to redo it?

2) I haven't send i693, so I guess the vaccination will be take care of when I have the medical examination done.

-Correct. Just find a Dr. annd do it.
