Question for J1

I have a question about J1 visa's Statement of Need letter. I am in the US on H4 visa now and just passed the USMLE Step 1 and Step 2 exams. In order to apply for J1 visa for the residency program I need a Statement of Need letter. Teh ECFMG Guide says the letter has to come from Ministry of Health of the Country but I knon nobody in Misistry of Health. How can I get a letter from them? Would the Depatement of State accept the letter from the hospital I worked before?
