You can only go to

under-served area to do your residency with J-1 visa if they want Statement of Need letter. Then after residency is done, you will need to work in under-served area for 5 years so Ministry of Health will apply for J-1 waiver for you. If you want green card afterwards, you probably need to stay in under-served area even longer time. This is a hard road, if you can find any other way, then avoid this.
