EAD申请的问题，急，在线等！

07年7月29日concurrently递了485同时递了EAD，批了以后一直没用，去年11月过期了，现在要用了想renew。麻烦的是，旧EAD卡和approval letter都找不着了。我现在能不能忽略以前申请过这个事实，不renew而是单独申请一个新的？我看了instruction以及皮老师关于EAD的博客，好像是可行的，我现在材料也是按上面说的准备的。在递之前，想就几个问题确认一下。我是属于交费的EAD，我准备的材料：
1. 765表（上面写的是申请而不是renew,没有A#,用的是I-94#）
2. 2张照片
3. 护照复印件（因为没有旧卡也没有批准信，485 receipt也没有，在律师手里）
4. $340支票
我不太确定是不是就这几样，请大家看一下，还有别的需要的吗？
