this is the worst situation you can get into it. You can file I-140 with a copy of PERM (not certified) together with a copy of letter to the Labor Dept. request for a duplicated copy of certified PERM. But this can take forever. You cannot file I-485 concurrently with I-140. You cannot request for Premium Processing for I-140 either. You can only wait till I-140 is approved (good luck on it - Labor Dept. will send the duplicate copy of ceriftied PERM directly to CIS. God knows when they will do it) then file I-485 when your priority date becomes current. I have a case exact like yours. I filed I-140 3 months ago and still no response.
