请问我见&假博士：I-485

请问我见&假博士：

In two weeks I need to be
interviewed by the local INS
for my I-485 application.

I was told by the interview notice
to have I-693A supplemental form
(varicella & TD caccine) with me.
I had physical examination on
May 21 2002. Can I ask that doctor
to send me a I-693A form? or I need
to have my examination again?
