No information on I140 yet. Is it possible to deny directly with

We sent applications to CSC in 09/29. AP approved, but no information on I-140 yet. ND is 10/07. Still waiting.
The problem is: my wife (H4) wants to go back China for two months soon. Is it possible that I-140 get denied directly without any RFE? Otherwise, she may come back using AP before the deadline for RFE, right?
Also, if I-140 is denied, will it affect her H4 visa application in China?
