The attendant may think you need I-94w form for being a Canadian citizen, however it is your responsibility to fill out I-94 which are everywhere in the inspection hall.

Now you may be considered "w/o inspection", as no document indicates you were inspected to be admitted.

Contact CBP ASAP.
