Thank you for your response. I did ask the attendance about the green form, she said no need.

Sorry I meant there's no expiration date on the stamp, but there is a date indicate the date of my entry. The immigration office did inspected my I-797 before she stamped my visa.

Theoretically I suppose they have my exit date and entry date.
