How to create a fillable PDF

The orignal file is a PDF file which contains lots of tables.I am wondering how to make this PDF file fillable by Adobe Reader 6.0 Professional so that I can fill that form on Acrobate Reader 6.0

It is quite urgent, any help will be highly appreciated
