(1) You need to use your notebook's audio output to feed your TV's audio input (L/R). Buy a special cable from Radio Shack or similar that has a stereo earphone plug on one end and two RCA male connectors on the other. You can connect the composite video out on your PC directly to your TV's video input using the cable that you already have.

(2) Video out only outputs video; no sound signal at all. See above for the cable needed.

(3) Yes. But for better results, you need a better TV.
