A more reliable data is statistics of deaths and injuries

Cars by Toyota is roughly 5 times safer than those by GM, 3 times safer than those by Ford, in 2008.

Those rare incidents of brakes are not reproducible in the laboratory condition. Therefore, it is very difficult to identity the exact reason. Most likely a system problem.
