Here is my experience. My doctor referred a physical therapist. I took treatments three time there and paid $10 co-pay each time (The total cost is around $290 each time). After that, my insurance refused to pay because the physical therapist is not in the next work. I called the administrator of the physical therapist twice. Finally they told me my doctor made mistake and I have no responsibility. They waived all charges.
