Check the service date on the insurance explanation of benefit

If it was last Wednesday when your daughter visited the doctor's office, it was billed correctly. Also check to see if it was doctor's fee or lab fee. Sometimes there is also facility charge. Just calling in to get a preion should not be charged. You can always call the insurance company and the doctor's billing office to figure out what the charge is and make corrections if any.
