Full name and addess of both party, sales amount. No witness is necessary. But signature of both party is a must. I would not put less amount than the actual amount to avoid future trouble. Trust me, do not take chance. The risk is not worth for just couple hundred bucks. Here is my personal experience. Recently I sold one car to an individual who put much less amount on the register sheet. The state government sent me a letter for verification. I have to tell the truth. I guess the buyer is going to have trouble.
