If your TV has ATSC tuner built-in (99.99% of them do), you don't need a set-top box.
Mine is a traditional VHF/UHF antenna, very big, with many branches, made by Winegard. I would not recommend the tiny ones from Bestbuy/Radioshack. Those are waste of money.
I'd recommend investing in a good splitter if you want to feed the signal to multiple rooms. I bought one for broadcast industry, <10, looks very crude but works much better than the fancy looking ones from Bestbuy/Radioshack. I have one feed going into a media center PC with an ATSC tuner card. It works like a TiVo.
