My simple solution

I usually add a horizontal brace near the top of the ladder to reach the middle of a ceilling

 - clamp two 2x4 to the two side of ladder near the top, adjust based on the target location

 - pad the end of the brace to prevent scratching

 - get another hand help raising the ladder

 - have someone to help steady the ladder since this configuration is not as stable
