Urgent question on 1099-misc

I hired an individula to do a work in 2011 costed more than 600. My understanding is I need to file a 1099-misc. Is this the right form?

I tried to downloaded it from IRS website. But I got the warning saying that I can't file the form A in red because it's not scanable. And I need to call IRS to order the form.

Also, it stated that form 1099-misc must file with 1096 together. What is this?
