Original Medicare: Parts A & B (1965)
These two parts were the foundation of the program and are still managed directly by the federal government.
  • Part A (Hospital Insurance): Covers inpatient hospital stays, skilled nursing facility care, hospice, and some home health care.
  • Part B (Medical Insurance): Covers doctor visits, outpatient services, preventive care, and durable medical equipment.
 
The Evolution of Private Options: Part C (1982–1997)
  • 1982: The Tax Equity and Fiscal Responsibility Act (TEFRA) first allowed Medicare to contract with private health plans.
  • 1997: The Balanced Budget Act formally established Medicare+Choice (now Part C), allowing beneficiaries to receive their A and B benefits through private insurers.
  • 2003: Renamed to Medicare Advantage under the Medicare Modernization Act. These plans often bundle hospital, medical, and drug coverage into one package.
 
Prescription Drug Coverage: Part D (2003)
  • 2003: Signed into law by President George W. Bush as part of the Medicare Modernization Act (MMA).
  • 2006: Coverage officially went into effect. It is a voluntary program offered by private insurance companies to help cover the cost of outpatient prescription drugs.
 
Supplementing the Gaps: Plan G (Medigap)
Medicare Supplement (Medigap) plans were standardized by the Omnibus Budget Reconciliation Act of 1990 to ensure consumers could easily compare policies from different insurers.
  • Plan G has become the most popular Medigap plan for new enrollees because it covers all out-of-pocket costs of Original Medicare (deductibles, copays, and coinsurance) except for the Part B deductible.
  • 2020 Milestone: Following a 2015 law (MACRA), Plan G became the "gold standard" for new beneficiaries as Plan F (which covered the Part B deductible) was phased out for those newly eligible for Medicare.
