Medicare B lowest monthly premium in 2025 is $185 and
highest is $628 (this is for a couple their income (MAGI) is $750,000+).
For New England area as an example, if a person buys Blue Cross Blue Shield MediGap (BCBS), monthly premium is $212 (better plan) or $129 (core plan), which covers all your medical needs, except $257 deuction for whole year. If you also buy Dental and Medicare D from BCBS, it costs $76 and $50 per month. Both have good coverage. So a montly cost of all these are $523 and $966, respectively. For a couple, yearly cost is $12,552 and $23,184. Here it uses MediGap plan with $212 premium / month to calculate.
Medicare D (drug plan) premium probably also depends on income. $50 premium is lowest one.
In New England, not sure how many states, but some states allow you to switch MediGap monthly between $212 plan and $129 plan. Difference between these two is $212 plan coveres Medicare A deduction (~$1,700 each admission to the hospital) and also covers more on skill nursing home, etc.
I don't think there're many people have income above $750,000.
