look at the form #1 and read it carefully which clearly stipulates that " If both birth parents are alive, and ONE or both of them will NOT be traveling with minor children, the non-traveling parent(s) must complete the form giving a notarized affidavit ofconsent to the person traveling with the child(ren) their authorization to take them in and out of the country". So, the form that I linked above is the correct form coping with your situation.
