Usually Congressman has his/her own website. You can Google to find your Congressman website, then go to his/her website directly and send your request to him/her from the website. That is waht I did.

Or, you may write a letter or just stop by his/her local office for help. Here is letter template for 485 for your reference.

His/her office will contact you and may ask you to sign and fax an authorization form to allow them to assist you.
