For car insurance, the penalty can be as serious as revoking your driver's license, i.e., your driving privilege.

For health insurance, the analogy would be revoking license of life (aka, death). Can the government do that and design a law to kill anybody without health insurance? Even if you say yes, I doub't anybody with sane mind would want that.
