Good question

I think they are exchangable at a lot of situations, but I can think of one case where they are not.

Let's say you and your friend went to shopping at mall, then
you lost your friend, you were looking around and
finally you found your freind, you'll say "oh, here you are", I
don't think you can say "here you go" at this case.
