Teacher Pjiang: Please help for EAD and AP

Dear Teacher Pjiang:

I am trying to renew the EAD and AP. My last two entries to US are as following:
2004 F1 at Los angels
2008 H1B from toronto.

I am confusing how to fill the EAD and AP

1: for EAD-last entry to US, I went through Homeland Security Custom Check at Toronto, how should I file where the last entry to US. Should I file Toronto or the city I arrived US.

2: for AP- class of admission, should I file F1 ( the first time I was admitted to US) or H1B( the lastest time I entry US).

BTW, don\'t care about some nosie made my mylawyer. Most people in this website recognized your help.
