Because you get your insurance from your employer, the administrative process is different than you shopping at a store.
you need to file an apeal, the first appeal goes to insurance appeal office. the second appeal goes to your employer. if you want to sue them in the court, you only can sue your employer, not your insurance because youe company has contract with them, you don't.
you can talk to HR also.
