I don't think you can use the old opt. Probably you will need to apply for another one. If the old opt should not have been issued in the first place, of course, you still can apply for it upon your actual graduation. Your case may be a little complicated because you or your school messed it up. Whether cis received the old opt card or not, they should have canceled it because you have not graduatd as required by law. When you file your application, you may request your school's international student office to attach a letter explaining what happened to the former application. By the way, I believe cis can check whether you have graduated on SEVIS system. Good luck.
