Unclaimed Property

Today I received a letter from California State Controller's office. It said that I may be entitled to the money from Delta Dental Of California. The reporting address is my very old address, 4 years ago. The letter mailed to my current address.

It asks me to fill the form with my driver license, SSN, and etc and mail to them to get the money.

The envelope is from John Chiang, State Controller, Di*****ursements Bureau. I don't know if this is a cheating or a real thing.
