Please help on a traffic collision

I was hit by a minivan from the back when I was waiting in an intersection. I felt uncomfortable and was sent to the hospital. Today I got the report from the police department. But there is no driver's license info and insurance info listed. What can I do? I only have liability insurance for this car. Thanks for any suggestion in advance.
