I have several questions about my tax, could you kindly give me some hints?

1. My H1 started in Oct.13, 2003, which form I need: 1040 or 1040 NR?
2. I just saw your post below talking about the non-profit company, I'm working in a university, does it count too? Is there any forms or deduction I can refer?
