I am on H1B and am filing w7 for ITIN for my parents.

Whose name shall i input here? My parent's?

Your parents’name.

Q3. 2. mailing address
my address or my parent's china address?

The ddress that you want the ID number mailed to, which I believe your current address in US.

Q4. who is the applicant? I or my parent? Since they had already left USA. Who should sign here?

Your parents are the applicants. They should sigh, but I am not sure what you should do since they are not here now.
