A Tax question ...............

I am a resident alien according to xxxx test. I have a personal account, but I did not get 1099 interest statements of 2004 from the bank. I already sent out the 2004 tax forms by the due time this year, how can I do?? I should ask the bank sent me 1099 form of 2004 or what else? Thank you very much!
