If you died while being married to a spouse, and 

(1) if the beneficiary was not your spouse, and there is no spousal waiver, the spouse automatically get 50% of the balance. 

(2) if the beneficiary was not your spouse, and there IS A SPOUSAL waiver, the beneficiary get 100%.
