问题是a的法律涵义

a panda, 是指一个熊猫，还是指熊猫类？

For example:

A panda usually roams up to 100 acres a day.这是指熊猫类。

A panda escaped the zoo。这是指一个熊猫。

那么，看看原文：

假设金钱数目不是问题，这里的a nurse是指一个护士（陪伴病人旅行），还是护士类？我想应该是护士类，否则假设一个病人需要（required by a patient)两个护士，就不能报了吗？

同样，"a parent who must go with a child"，这个parent如果按父母类而不是一个父母理解，也说得通。一般做重大医疗决定需要两个父母同意，虽然可以打电话，一般孩子到外地去看病，两个父母跟着去，可以说是must。
