牙医不能提供证据，保险公司拒付， 如何应对？

documentation of disesea or injury to consider this service. Therefore, no benefits are payable

......

已经要求牙医再提供更多文件。他不理睬。没有保险公司这几百美元，我还要欠他钱。他不停来信催款。

就拔一颗牙，收了1500美刀。太黑了。
