733.90 "Disorder of bone or cartilage" is not preventive.

77808 is not a valid CPT code.  It might be 77078 CT Bone Density or 77080 DXA boen density.

May need to add CPT code 99396 "Preventive Visit - New Patient age 40-64" or 99386 "Preventive Visit - Established Patient age 40-64".

Insurance Company is right about not letting you know what codes they are looking for.  Just like the teacher won't let students know the answer for an exam.  To prevent fraud.
