After issuing the offer

In my mind, the most professional way is to tell them, if they would issue you an offer and conduct a background check then you can give them the contact information for your current manager or most recent one.  In face, the most recent one does not matter unless s/he is the current one as well.  Just put "NO" there will appear not as a professional attitude.
