question about opt and H4

I'm holding opt (f-1 visa) right now and it'll expire next January. My hu*****and is holding H-1 visa. I'd like to change my status from F-1 to H-4 after opt is expired. How can I do that? What documents are needed? Also, if in the future I want to change from H-4 to F-1, is it possible?
