Question on I-485

Part 3 " Were you inspected by a US immigration Officer? " Yes, No ?

I entered in the US as a F2 visa then Change to F1 here. So the "nonimmigration visa number" here should it be my F2 visa number or the F1 status I94 number? " " date visa issued" be my old F2 visa date or the date that change to new F1 satus?

Really confused. Thanks for any of your help.
