I-485填表请您把把关

请问前辈，我正在准备I-140和I-485一起file EB2NIW. 同时我还替我的太太file I-765. 填表时我有几个问题没把握，敬请前辈把关：

Form 750b1:
10.b. Alien is in the United States and will apply for adjustment of status to that of a lawful permanent resident in the office of the Immigration and Naturalization Service at
City: State:
这里的城市是填我居住的城市还是递交申请的移民处地址？

Form I-765:

Eligibility under 8 CFR 274a.12 ：我太太目前是用J-2拿的工卡，原来填的是(c) (4).
现在和我们的I-485一起file, 性质应该变了，是否填(c ) ( 9 ) ？另外，在
I am applying for: 一项，是否应该选“Permission to accept employment.” 而不是“
Renewal of my permission to accept employment“？

I am applying for an adjustment to permanent resident status because:
a. an immigration giving me an immediately available immigration visa number has been approved….

h. Other basis of eligibility

我可以选a吗？如选b，怎样填好？
