A nurse for H1 without TSE?

My wife passed the RN exam(NCLEX) last year and she filed her I-140 and I485 one year ago.
So far, she didn't pass the TSE/IELTS (because it's really hard to get TSE 50/IELTS(7.0 for Spoken English)). Maybe, it needs more time to reach the score.
However, our I-485 is still in the process. So, what shall we do? Can she apply for H1 now?
By the way, I'm F1 now, and she is working with her EAD.(Her I-140 was approved in March)
Can anybody tell me sth?
