The original I-797 USCIS sent to the employer should be a full 8" x 11" size. The upper part is the approved petition, the lower part is 1. the approved change of status (that is, I-94) or 2. if consular notification, no I-94 and no approval of H1 status. The upper part will also state the COS approval status.
That's why many US consulates do want to see the upper part at the H1 visa interview. The lower part is useless once you leave US (unless you go to Canada and want to return in 30 days using that I-94).
