1，是不是有2份东东要分别寄给联邦和州府，
一份里面是w-7,1040表，W－2表和H4的护照或公证，，还有其他东西吗？
一份寄给州税的就是一个表，和w-2表，是吗？
是否还有补充的材料要寄，w-2表一定2个都要寄是吗？

Yes, you will have to mail to Federal and State separatly, enclose your W-2s(you should have 4 copies of W2s, one for yourself, 1 for Federal, 2 for state(s)). Depends on your case, you may need to attach other documents e.g. schedule A and form 8863 if your wife is a student. Be careful with your W-7, if you made any mistakes there, it will delay your return check for a long time.

2，由于H4没有SSN和税号，那么报州税那张表好像那一栏填此项目的是不是就空着？

Yes, leave it blank or put "TIN-TBD" there.

3，有人是否知道办护照公证的程序，是不是很麻烦，要花多少费用呢？

No cost, just take the copy to your bank or a notary public and they will notarize it for you for free.
